BCBS doesn't pay when it should part 4

The BCBS site shows:
Total Deductible to Meet: $1,500.00
Total Deductible Met to Date as of 10/29/2009: $1,516.26
Total Deductible Remaining: ($16.26)
But when you add up the member liability amounts
$ 1,249.73 (for medical claims (double checked)) and
$ 263.80 (for RX claims (double checked))
$ 1,513.53 is the total you get.
These are the same figures as last time I looked.
I do not know what is going on...
I haven't received an email from the BCBS employee letting me know everything has been straightened out.
Maybe it's not still. Next Tuesday (November 10th 2009) will have been 3 weeks. Maybe it will be fixed by then.

12-03-2009 8:47am I checked the BCBS website to see how the corrections they made worked out.
Since I never did received that detailed email (nor phone message) of what they did.
Guess they never bothered to finish adjusting it. It's still incorrect.

Total Deductible Met to Date as of 12/02/2009: $1,513.53
Total Deductible Remaining: ($13.53)
Which equals the member liability amounts when you add them up. It hadn't last time I had checked.
$1,249.73 Medical Member Liability Amount
$ 263.80 Pharmacy Member Liability Amount
added together equals
$ 1,513.53 for the total Member Liability Amount. Which is the same as it was last time I checked.
So they owe me $ 13.53

I advised her:
Total Deductible Met to Date as of 12/07/2009: $1,513.53
Which still needs to be adjusted for the $13.53 since my deductible is only $1500.00
12:40pm she placed me on hold to look into it.
She came on the line a few minutes later and said she saw on her end that there is still an open case since October 20th.
She will send them an email (who ever "them" is) advising them that this is a priority to close/resolve.
I provided her with my phone number and gave her permission to leave me a voicemail once she gets a status.

Verizon bills wrong account

I have Verizon FiOS Internet.
I do not have phone or TV service through them.
I do not receive a bill in the mail from them. Everymonth it is automatically debited from my bank account/credit card.
I am switching banks.
I have no idea what my account number is with Verizon. So I had to call them.....

2009-11-23
12:36pm called 800-VERIZON (800-837-4966)
Male answered after many minutes on hold and was able to pull it up my account with my name and address. I provided him my new credit card info.
He gave me a confirmation number: XX-00000000000
He said I had 2 accounts on file and didn't know what account it was for. He just updated my billing info only.
The active account number could be either
0000000000000
or
X000000000 (odd that there both different sizes….)
He advised me that I'll be able to sign in with those numbers and when I sign in online that I would need to know that 10/19 or 11/19 is my last billing date.
He tried to upsell me FiOS TV for an additional 55 monies a month. I told him that was way too much. He stated in a shocked voice “You think $55 is too much a month for TV??!” I told him “Yep!” Netflix is $9 bucks a month.

When he tried to upsell me internet speeds he couldn't give me the advertised online price. He also said I would need a one year agreement if I wanted the faster plans (or pay more a month with out having a commitment). I declined due to that.

I have an older bank account that I'm slowly migrating away from.
I don't have very much money in there due to that.
I just noticed it was overdrawn this AM.
Turns out Verizon billed that account even though I had called them 2 Mondays ago and provided them with my info for my new account.
Grrrrr. Looks like I will have to call them at lunch today...

BCBS doesn't pay when it should part 3

The site states:
Total Deductible to Meet: $1,500.00
Total Deductible Met to Date as of 10/29/2009: $1,516.26
Total Deductible Remaining: ($16.26)

BUT when you add it up it comes out to:
$1,249.73 Medical Member Liability Amount
$ 263.80 Rx Member Liability Amount
$1,513.53 total Member Liability Amount

So..... Still not full fixed. But it's getting there.

BCBS doesn't pay when it should part 2

2009-10-20
4:30pm Called "Member Services" at 1-800-XXX-XXXX
Selected Option 2 then speak what you are among the following options I said "policy holder" (is where I fell. I’m not a dr…). It didn't understand me. I just stabbed the "0" and the "#" keys till it said it would transfer me to a representative.
4:32pm listening to hold music from the 1920's.
4:33pm John answered.
4:34pm I fully explained my issue to John.
4:35pm John doesn't think anything was changed.
4:36pm CORRECTION He says that it looks like on the internal site (there side) that they may have started making changes and are just waiting for one big adjustment to go through.
He stated that it can take time for them to show up correct on the external (the site I view) site.
He says that there is a total of $1632.11 in claims/Rx on there end being applied to my 1,500 deductible. This is even more than the amount on my end.
4:37pm He placed me on hold while he researches this to see what MEDCO has (whatever that means. Is that an internal system or a different insurance company?).
4:39pm He came back and advised me that they were supposed to do a mass adjustment.
But that it’s not showing that it's done on there end yet.
He's going to send it to there adjustments department and have them take care of it, by adjusting previous claims to get it down to the 1500 deductable.
When that is done if they were claims that have already been paid I would need to ask the provider for the refund (from those paid claims)
Which sounds like a nightmare to me.
Possibly even illegal since this is HSA monies were talking about.
I asked John how long this will take. He says it can take 2-3 weeks. He will call me and leave me a detailed voicemail of what claims have been adjusted.
I asked if he could email he said he will do that instead.

BCBS doesn't pay when it should

2009-10-06
Backstory:
I have the High Deductable Health Plan (HDHP).
There is a $1,500 deductable then everything pays out at 100%.

I just noticed that the values aren’t adding up.
According to EOBs on the BCBS website I have
$1,365.58 in medical claims
$236.45 in Rx claims
$1,602.04 combined total of medical and Rx claims. Which is $102.04 over the $1,500 deductible.

2009-10-06 12:02pm called and spoke with Quinn at Corporate Employee Benefits
1-800-XXX-XXXX.
After I explained the situation to her she advised me to call "Member Services".
She placed me on hold while she looks for a phone number for me to call.

2009-10-06 12:10pm
Called "Member Services" at 1-800-XXX-XXXX
12:13pm - April answered
12-14pm - she put me on hold to figure out what is going on.
She came back on the line. Advised me that she see's the error and that I should call back in a few weeks and it will be resolved hopefully.
12:19pm End call

2009-10-13
Went online to BCBS website and added up the amounts on the EOBs to see if hopefully it has been resolved.
$1,365.58 total for medical claims showing as “Member Liability Amount” (checked math twice)
$263.80 total for Rx claims for “Member Liability Amount” (also checked the math twice)
$1,629.38 total which is $129.38 over the $1,500 deductible.
Will check again next week at the 2 week mark.

BCBS website is still showing my total
"Member Liability Amount" for medical claims as:
$ 1,365.58
"Member Liability Amount" for pharmacy claims as:
$ 263.80
Which is the exact same as it was last week.
Looks like I will have to give them a call today.

AT&T's exclusive iPhone fails again

I haven't been able to use the internet or text or mms at all today.
Maybe the iPhone OS update update made it better for a few days. But sadly it's just as bad as it was.
I will be stopping by the official AT&T store after work today to get a new sim card.
Even though I don't think it's anything to do with the hardware (But rather something on AT&Ts network side) I'll follow there recommendations.

AT&T/Apple and the failing iPhone

2009-10-09 Thursday

12:27pm Called AT&T Customer Service (AGAIN) at 1-800-331-0500
12:29pm Andrea answered. She took my info and reviewed me notes when I asked for there "tsd" department.
12:31pm she was able confirm what I said. She placed me on hold to get a "tsd" rep on the line.
12:34pm Charles from "tsd" is warm transferred over.
12:35pm Charles stated that since I've already done a restore (in iTunes) and that didn't resolve it that it is an issue the phone itself and that I will need to speak with Apple. Provided me the phone number (800) MY-IPHONE and is transferring me to them.
12:37pm I'm going through Apple's ivr.
12:37pm Victor (Apple rep) answers
12:44pm placed me on hold while he's researching my issue on his end.
12:49pm he came back and is transferring me to a new rep (without doing any troubleshooting).
12:50pm Raven (tier 2 at apple) tries calling me. My phone never rang and she just goes straight to voicemail.
12:55pm Raven stated that I am out of the warranty and if I need a new phone I should have had the Apple Protection Plan Program. I advised her that I have only had this phone for 2 months. Only 1.5 of them without any problems. She said she'll see what she can do.
12:58pm Raven says the first thing I should try is to get a new sim card after work to see if that solves my issue (or if I notice any improvements whatsoever with a new sim). She will email me the case number.

A few mins later I did get her email with the case number in it.

1:54pm seem to have working internet. I was able to even browse the app store and purchase a game (FROGGER!!!!). Why is it working suddenly now?Sadly about 20 mins later started having the same issues again.

5:00pm Read on engadget that the new iPhone OS is out. Downloaded it and noticed in the release notes they talk about fixing network connection issues (which sounds exactly like what I've been suffering through since the mms carrier file was released).
